 What information do I need to complete the form?

You will need the following details:

Club Information:

    • Club name
    • Club type (e.g., youth, adult, mixed)
    • Contact address, including postcode

Primary Contacts:

    • Chairperson’s name and contact information
    • Secretary’s name and contact information
    • Treasurer’s name and contact information

Welfare and Safeguarding:

    • Welfare Officer’s name and qualifications (mandatory for youth clubs)
    • Confirmation of adherence to FA safeguarding policies

Additional Information:

    • Details about any existing or affiliated teams
    • League the club plans to participate in (if applicable)
    • Supporting documents (e.g., constitution, meeting minutes).

What happens after I submit the form?

  1. Once submitted, Derbyshire FA will review your application.
  2. You will receive an acknowledgment email confirming receipt of your form.
  3. If additional information is required, the team will contact you.
  4. Approval can take 5–10 working days.
